PATIENT RESPONSIBILITIES

  • Must bring insurance card to every visit
  • Must pay co-pay at every visit (Visa, Mastercard, Discover accepted)
  • Bring a list of all medications you are currently taking
  • Patient must verify insurance coverage for complete physical exams
  • Patient must stop at the front desk when leaving to:
     1. Make a return appointment
     2. Get a referral for a specialist
     3. Get new prescriptions
